Icon Manager icon archive
From Just Solve the File Format Problem
Icon Manager icon archive (.ica) is a format associated with Icon Manager, an application for Windows developed by Leonard A. Gray and Impact Software. It stores icon images, and associated metadata.
Icon Manager is mainly for Windows 3.x, though there is at least one beta version for Windows NT. It's not clear if Microangelo, the apparent successor to Icon Manager, supports the format (guessing no).
There are at least two versions of the format.
